I want to use neural network toolbox for the medical diagnosis.

I have symptoms of various diseases. I don't know how to use them in the form of matrix as I have given every symptom variation scale as Mild/Severe/Very_Severe. I am confused how to give Input matrix and Target matrix for the symptoms that are varying over a various scales.

I have succeeded in using this in fuzzy logic toolbox. But I want to extend this using neural network toolbox and then combine these two toolboxes making it a neuro-fuzzy model for medical diagnosis.

Symptoms: Temperature, Headache, Joint/Muscle Pain, Abdominal Pain, Vomiting, Dark Urine, Yellow Eyes, Cough, Appetite, Chest Pain, Chills, Dehydration, Cramping, Pain Behind Eyes.

Diseases: Viral Fever, Dengue Fever, Hepatitis, Tuberculosis, Malaria, Diarrhea.

Please give me a detailed explanation.
