Hodgkin-Huxley Error Code: Index exceeds the number of array elements (1)

Can someone please help me figure out my my code will not work. I am not understanding how to fix the error.

Your differential equation returns a column vector of 4 elements, so it is necessary for the initial conditions vector and whatever ‘y’ is in the other screen capture to have 4 elements as well.
