Step By Step Simulink Coder -> Library -> C++ Application

Can anybody point me to an information source for this? I believe I have Library created but when I try to utilise in C++ the definitions and methods do not seem intuitive to use.
Alternatively, does anybody know of an end to end tutorial on this?
