How can I get rid of the unwanted pixels circled in red in the image below?

Fill holes and then bwareafilt and then "and" the result with the original to get the holes back again.

Use .* to implement the "and"

fBW = imfill(BW,'holes');
fBW = bwareafilt(fBW, 1);   %keep largest only
nBW = BW .* fBW;            %remove holes

I think it doesn't work because there's only one region. I confirmed it with regionprops.

fill, then thin the image (erode once). That should disconnect, I think. Then bwareafilt, then dilate by one pixel, then multiply the result by the original image to restore the holes.
