Statistical CSI vs Instantaneous CSI

Hi
I want to know the exact difference between Statistical CSI and Instantaneous CSI. How they affect (or how they are reflected) in the equation of SNR/SINR?
Lets suppose the SNR at destination is (h^2 *Ps)/d^a). Where h is the channel gain and is generated using
h = 1/sqrt(2)*(randn+1i*randn); %Rayleigh fading channel
and Ps is the source Tx power, d is the distance between source and destination and a is the path loss exponent.
How is throughput calculated in both cases? (e.g. can I use Shannon capacity eq for both)?
Any kind of help will be really appreciated.
