How can I connect a nut to a leadscrew to apply torque to rotate it?

I would like to create a mechanism to apply torque to the lead and nut of a lead screw and add them together to rotate. How can I connect the nut to the lead screw to apply torque to rotate the lead screw, as shown in the first image? I am using Simscape Multibody. I am referring to the model in the second image.

If you wish to apply torque instead of force, change the External Force and Torque Source to apply "tz" instead of "fz". Make sure the External Force and Torque Source is connected to a frame which can rotate.
