SQP algorithm crashes showing "The number of output should match the number of inputs"

I used fmincon with symbolic toolbox providing gradient and Jacobian of my objective and constraint functions respectively, when I used "interior-point" algorithm for fmincon, I can get the results. But when I changed algorithm to "sqp" (optNLP = optimset('Algorithm', 'sqp', ...)), the code crashes and showing "Error using deal (line 37), The number of output should match the number of inputs", following with some other indications of errors. I don't know why it happens and what could be done to deal with it. Very grateful if anyone can help.

Please give use your entire fmincon call and entire optimset call.
