How to use Listener for plot data to App Designer graph from SIMULINK model when simulating ?

Dear experts, I'm new to listener and I find it quite hard to use it for extracting data from SIMULINK model (when simulating it) and plotting these data to a graph in App Designer. Could anyone have a tutorial (step by step) for this problem? Thank you in advance.

But how could I use it for App Designer, I try but fail to transfer data from Simulink to App Designer Graph. Could you give me an example for that? Thank you in advance.
